The National Medical Commission (NMC) has capped MBBS tuition at 4.5 years. MBBS stands for Bachelor of Medicine and Bachelor of Surgery. An NMC public notice dated April 7, 2026 confirmed this rule. Colleges cannot charge tuition during the one-year Compulsory Rotating Medical Internship (CRMI). The order covers every government, private and deemed medical college in India.
Tuition is only one part of the actual bill. Hostel, mess, caution deposit and university charges still apply. These are billed for the full 5.5 years of the course. The gap between tuition and total cost runs into lakhs.
Why Tuition Fees Do Not Equal Total Fees
Tuition fees cover only classroom teaching and clinical training. Every other course-related cost sits outside this figure. The MBBS course runs 5.5 years in total. Only 4.5 years involve academic study under NMC rules. The final year is the CRMI, a supervised service period.
During the internship year, tuition is now zero. But hostel and mess bills continue for that year. Books, exam fees and personal costs also stay. This is why the total bill always exceeds the tuition figure.

Hostel and mess bills are the biggest hidden cost. Karnataka private colleges charge Rs 70,000 to Rs 1.5 lakh yearly. Maharashtra private colleges bill Rs 1.5 lakh to Rs 3 lakh yearly. Government college hostels are far cheaper but still add up.
